To submit a District Salute Proposal, follow these EASY steps:

After you complete the form, print out two copies of the form for your records.
Once you have finished printing two copies of the form, click on the Submit button to submit your proposal online.
Please mail us your technology plan along with a printed copy of this form (materials will be accepted but will not be returned). Please mail a copy of your materials to:
